Forms and Insurance

It is common to have questions about the use of insurance for behavioral health benefits.  It is important to be aware that billing insurance means some limits to privacy.  However, once you have decided that using your insurance is a comfortable choice, I am a provider to a variety of insurance panels.

Insurance Accepted:

        Tufts Insurance (all panels)

        Blue Cross / Blue Shield of MA  (all panels)

        Harvard Pilgrim Health Care / United Behavioral Health

        All Indemnities

        Out of Network

Accepted payment options: cash and check.

Cancelation Policy:  Kindly allow 48 hours for cancellation notifications, otherwise you will personally be billed my full fee.  Your insurance can not be billed for missed or late arrival appointments.
